the airline pilot and the service form
How the airline pilot used those 'tell us how you liked our service' cards to make a point with his management.
An airline pilot, upset because he had been placed on doing freight runs, took the names off of the coffins that he carried. He then obtained some of those “Tell us how you liked our service” forms. He used the names of the corpses, filled out the service forms complaining that the service was poor and that hey would never ride this airline again, and sent them to the vice president of his company.
